Sometimes, a small change makes the biggest difference. From smart devices and mobility aids to essential home upgrades, we help you access the Assistive Technology and Home Modifications (AT-HM) you need to stay in the home you love.
Our team guides you through the government funding process to ensure your home remains safe, accessible, and comfortable for your future.

Maintain your independence with the right tools. We source everything from essential mobility aids (like walkers and scooters) to daily living supports, such as shower chairs, dressing sticks, and personal alarms, to make everyday tasks easier and safer.

Whether you need a simple grab rail installed in the bathroom or a major modification like a wheelchair ramp, stairlift, or accessible shower renovation, we coordinate reliable, licensed tradespeople to adapt your home structure to your changing needs.

Your dedicated Care Designer, who is a Registered Nurse, works closely with an Occupational Therapist to ensure every modification fits your personal needs and goals. We guide you through the assessment process and help with the paperwork.
Under the government’s Support at Home program, eligible participants can access the Assistive Technology and Home Modifications (AT-HM) Scheme.
This scheme provides upfront, separate funding, meaning you can access essential safety equipment, from simple grab rails to smart safety devices, without using up your daily care budget.
Eligibility is determined by your standard My Aged Care assessment. If you are being assessed for Support at Home, you will automatically be evaluated for the AT-HM Scheme. If you already have a care plan but your needs have changed, we can help you request a re-assessment to access this additional support.
While the government provides an allocation to cover these costs, a small co-contribution may apply depending on your financial situation (e.g., self-funded retiree vs. pensioner). We will always provide a clear, transparent quote before any work begins so there are no surprises.
Feel secure in the home you love. Simple additions like grab rails, non-slip flooring, or smart sensor lights will significantly reduce the risk of falls, making moving around your home easier, safer, and more comfortable, so you can live confidently at home.
Maintain your freedom to do what you love. The right mobility aids and equipment can help you manage daily tasks on your own, giving you the confidence to get out of the house, connect with your community, and join in social activities.
Reduce the need to rely on family or carers for everyday things. Knowing you have the right support in place, from personal alarms to automated devices, gives both you and your loved ones greater peace of mind and allows you to focus on enjoying your time together.
A flexible solution offering a full comprehensive suite of services tailored to your specific needs. We assist with personal care, meal preparation, companionship and more, ensuring you have the support required to live in your own home, on your own terms.
Clinical support delivered in your own home. We use qualified Registered and Enrolled Nurses to manage wounds, injections, and medication, providing hospital-level expertise to keep you safe and comfortable in your familiar environment.
Specialised support for disability participants. We offer flexible services to build capacity and access the community, ensuring Self or Plan-Managed participants achieve their goals with full choice and control.
We are a government registered Support at Home aged care provider and can help you navigate the system to access this funding . All care is overseen by a Registered Nurse who will design a flexible care plan for your unique needs - this ensures you have professional support.
The AT-HM Scheme is part of the Australian Government’s Support at Home program. It provides separate, upfront funding for safety equipment and home upgrades, meaning you can access the tools you need to stay safe without using up your regular daily care budget.
Eligibility is determined during your standard My Aged Care assessment. If you are assessed as needing support to live independently, you may be approved for AT-HM funding. If you already have a Home Care Package but your needs have changed, our Care Designers can help you request a re-assessment to access this scheme.
The funding covers a wide range of items designed to keep you safe and independent. This includes:
No. A key benefit of the AT-HM Scheme is that the funding is separate from your quarterly care budget. This ensures you don't have to choose between getting a safety rail installed and receiving your weekly nursing or social support visits.
For simple items, you may not need one. However, for "Medium" and "High" tier modifications (like structural changes or complex mobility aids), the government requires a prescription from a qualified health professional. Your Care Designer will coordinate this for you, connecting you with a trusted OT to ensure the modifications meet both clinical standards and your personal lifestyle goals.
The government provides the bulk of the funding, but depending on your financial situation (e.g., if you are a self-funded retiree), you may be asked to pay a small co-contribution. We will always provide a transparent quote before any work begins so you know exactly what to expect.
Dealing with Kristy has been efficient, timely and every question I have had has been answered without a hiccup. Care of our Mother is one of the most important and stressful things we have had to do as her children but we feel supported by Nurse next door to treat and care for her as we would, actually they are probably better at it than us. Mum has had Lily, Meg and Eva, coming in to care for her of the morning and she is noticeably happier and more comfortable for the day. I would highly recommend Nurse next door it has made a huge difference for Mum and for us.
We have recently engaged Nurse Next Door, I really liked the approach Lauren had when meeting my Mum. The focus was on finding out what she has stopped doing and would like to start again, rather than being all about the medical side of things. Both Tim and Lauren spent a lot of time talking to me about Nurse Next Door before we made a time to meet. I really appreciated the information they provided and the obvious care they both had with helping my Mum and also me.
Nurse Next Door from the initial visit till my Mum passed were outstanding, caring carers is the best way to describe them, i can recommend them highly if you have the need for carers. They are with the loved one the whole time allocated and do the work that is required and more, they are above and beyond.
My wife & I highly recommend Sarah and her team at Nurse Next Door Bundaberg. My wife's sister fell and injured her knee. She lives by herself - Has no car and family support does not exist. She was unable to walk. One phone call from us and Sarah was there working out a plan. She was wonderful and greatly appreciated. Thank you, Thank you for all that you have done. You are an angel.
I would like to commend Nurse Next Door on their wonderful service especially Taylor who looks after my Father John Hewitt her care and compassion towards Dad is excellent and communication with Dad's care is exceptional, the care and compassion from Nurse Next Door is second to none thankyou
We have been extremely lucky to have Jess and Zoe as our care providers. I am very fussy when it comes to carers for my sons but I could not ask for a better fit than Jess. Jess has been a god send to me, as a single parent to two boys with ASD, I find it hard to do many social activities with the boys as their day to day care needs are quite high. Jess takes them fishing, shopping and for a bite to eat. Jess is vital in ensuring the boys learn about how to participate in their community and teaches them vital skills such as using money, asking for things and appropriate behaviour in public. Jess looks like any other person getting around with the boys and blends in perfectly as my sons do not want others to see them with a 'carer' wearing a uniform and advertising this role and their autism. This is essential in helping the boys feel accepted in the world. Jess is rapidly becoming part of our family and the boys love him. He's cool, professional and relates well to my boys and I. I feel very safe knowing the boys are with him. I cannot recommend him enough. Zoe is critical in coordinating the boys care needs with Jess and is always polite, astute and reliable. She is indispensable to me and my boys. Cannot recommend these lovely people enough. Thank you Jess and Zoe from a very grateful Mum.
Nurse Next Door stepped in to assist with urgent, in-home palliative care when our regular provider failed. They provided a responsive, compassionate and professional service that permitted my mother to die in her own bed, with loved ones around her as she wished. As a health professional myself, based on our experience, I fully recommend them.
I can only recommend! Great service! Shalini was punctual, communicative, compassionate and knowledgeable. Exactly what I needed after my hospital recovery. Many thanks!
My family cannot rate the care and attention supplied by Michael and his team highly enough. We were in urgent need of in-home social, domestic & medical services for our 93 yr old mother. Nurse Next Door were able to respond almost immediately to our enquiry and provide exactly the right people for the job - showing great sensitivity for the challenges of dealing with a somewhat reluctant elderly client and soon building trust with her. Michael's flexibility and willing adaptability to meet our mother's frequently changing circumstances (due to her fluctuating health) were remarkable and the dedication of his staff to my mother's health and well being was truly remarkable. We could not recommend the services and client management shown by this team more highly - in every respect Nurse Next Door is an outstanding service provider.
I 💯 recommend Door Next Door Service Provider to friends, families and to the Community. Since my mom is with this Service I am satisfied with the work and support. The Coordinator always follow up with the support workers and service.
