Your dedicated Care Designer, who is a Registered Nurse, works closely with an Occupational Therapist to ensure every modification fits your personal needs and goals. We guide you through the assessment process and help with the paperwork.
Under the government’s Support at Home program, eligible participants can access the Assistive Technology and Home Modifications (AT-HM) Scheme.
This scheme provides upfront, separate funding, meaning you can access essential safety equipment, from simple grab rails to smart safety devices, without using up your daily care budget.
Eligibility is determined by your standard My Aged Care assessment. If you are being assessed for Support at Home, you will automatically be evaluated for the AT-HM Scheme. If you already have a care plan but your needs have changed, we can help you request a re-assessment to access this additional support.
While the government provides an allocation to cover these costs, a small co-contribution may apply depending on your financial situation (e.g., self-funded retiree vs. pensioner). We will always provide a clear, transparent quote before any work begins so there are no surprises.
