Your dedicated Care Designer, who is a Registered Nurse, works closely with an Occupational Therapist to ensure every modification fits your personal needs and goals. We guide you through the assessment process and help with the paperwork.
Empowering your Independence
Under the government’s Support at Home program, eligible participants can access the Assistive Technology and Home Modifications (AT-HM) Scheme.
This scheme provides upfront, separate funding, meaning you can access essential safety equipment, from simple grab rails to smart safety devices, without using up your daily care budget.
Eligibility is determined by your standard My Aged Care assessment. If you are being assessed for Support at Home, you will automatically be evaluated for the AT-HM Scheme. If you already have a care plan but your needs have changed, we can help you request a re-assessment to access this additional support.
While the government provides an allocation to cover these costs, a small co-contribution may apply depending on your financial situation (e.g., self-funded retiree vs. pensioner). We will always provide a clear, transparent quote before any work begins so there are no surprises.

What is the Assistive Technology and Home Modifications (AT-HM) Scheme?
The AT-HM Scheme is part of the Australian Government’s Support at Home program. It provides separate, upfront funding for safety equipment and home upgrades, meaning you can access the tools you need to stay safe without using up your regular daily care budget.
Am I eligible for home modification funding?
Eligibility is determined during your standard My Aged Care assessment. If you are assessed as needing support to live independently, you may be approved for AT-HM funding. If you already have a Home Care Package but your needs have changed, our Care Designers can help you request a re-assessment to access this scheme.
What can I buy with AT-HM funding?
The funding covers a wide range of items designed to keep you safe and independent. This includes:

- Home Modifications: Installing grab rails, wheelchair ramps, stairlifts, or bathroom renovations (like level-access showers).
Will using this funding reduce my hours of care?
No. A key benefit of the AT-HM Scheme is that the funding is separate from your quarterly care budget. This ensures you don't have to choose between getting a safety rail installed and receiving your weekly nursing or social support visits.
Do I need an Occupational Therapist (OT)?
For simple items, you may not need one. However, for "Medium" and "High" tier modifications (like structural changes or complex mobility aids), the government requires a prescription from a qualified health professional. Your Care Designer will coordinate this for you, connecting you with a trusted OT to ensure the modifications meet both clinical standards and your personal lifestyle goals.
Do I have to pay a gap fee?
The government provides the bulk of the funding, but depending on your financial situation (e.g., if you are a self-funded retiree), you may be asked to pay a small co-contribution. We will always provide a transparent quote before any work begins so you know exactly what to expect.
