Starting 1st November 2025, the new Support at Home program will replace the current Home Care Packages (HCP). This program is designed to make it easier for older Australians to access the right care and support, including services, equipment, assistive products, and home modifications. The goal is to help people stay healthy, independent, and connected to their community. If you’re already receiving a Home Care Package, you’ll be automatically moved into an equivalent Support at Home package with no disruption to your care.

The Support at Home program introduces a simpler and more flexible way to manage your care compared to the current Home Care Package system. Instead of one annual budget, you’ll receive quarterly funding that aligns with your support level. This makes it easier to plan and manage your care needs throughout the year, with funding divided into four payments.

If you’re already receiving a Home Care Package (HCP), are on the National Priority System, or were assessed as eligible before 12 September 2024, you won’t be disadvantaged under the new Support at Home program. Your financial contributions will stay the same, or may even be lower, than under the current HCP system. You’ll also continue to have access to residential aged care under the existing fee structure whenever you need it.
If you were assessed for a Home Care Package between 13 September 2024 and 30 June 2025, you’ll be considered a transitioned participant. This means you’ll receive a Support at Home package equal in value to your current HCP, with some contributions applying for non-clinical services.
This term is being used by the government to refer to a person with an existing Home Care Package approved prior to September 12th, 2024 which will allow them to continue to receive the same benefits under the old Home Care Package rules.
This means your existing funding is protected and the Australian government guarantees that you will be no worse off under the Support At Home contribution arrangements.
When Support at Home begins, if you have accrued any unspent Home Care Package funds, these will transition across with you.
A co-contribution is the amount you may need to pay towards the cost of your care services under the Support at Home program. Instead of covering the full price of services, the government subsidises most of the cost and you contribute a smaller share.

In addition to these three categories, the program also provides separate funding for Assistive Technology and Home Modifications (AT-HM). This can include mobility aids, equipment, or home changes like grab rails or ramps to make living at home easier and safer.
If you or your loved one is unhappy with the quality of home care or the current provider, you can request a change of providers through My Aged Care at any time, and your current provider must support you in the move.

Tip: You don’t need to wait for the annual review. If you notice changes in your health or independence, it’s best to request a reassessment early so your support can increase as soon as possible.
