What is a Care Designer?
A Care Designer is always a senior Registered Nurse. They are your dedicated partner who develops and oversees your care plan, manages the roster, and ensures we are meeting your specific goals, whether that is remaining independent at home or accessing the community.
How does Care Management work with the new Support at Home Program?
Under Support at Home, Care Management is a vital service to help you coordinate your distinct funding budgets (Clinical, Independence, and Everyday Living). We help you plan your quarterly budget so you don't run short on essential services.
Can I access Care Management privately?
Absolutely. Many families choose to pay privately for a Care Designer to oversee a loved one's care, especially if they live interstate or have complex medical needs. We provide the same level of clinical oversight and advocacy regardless of how the service is funded.
Can you support both NDIS and Aged Care in one household?
Yes. Mixed funding households are common. If one family member is on the NDIS and another is accessing Support at Home, your Care Designer can coordinate rosters to minimise disruption and streamline communication.
Do you charge for Care Management?
Yes. For Support at Home clients, this is paid from your funding budget. For NDIS participants, it is covered under relevant support items. For private clients, we offer a transparent fee structure. We ensure you understand all costs before care begins.
What happens if there is an emergency after hours?
The Care Services Team is available 24/7, 365 days a year. If a crisis occurs or urgent advice is needed, you can call and speak with a team member who knows your care plan.

I have been made welcome, appreciated, and adopted by each family member of Nurse Next Door who communicate and care for me. I love how extensive my individual care concerns are acted upon whether caring for me paid time or when careers are in their own time. As a diabetic I rely on medication and pen for injections each week. I in conversation communicated that supplier were not able to keep up with Diabetic suffers demands, next moment Belinda was phoning many chemists to see if they had supplies? Eventually a chemist one hour round trip from my location had one in stock. After leaving details Belinda prepared to drive to the chemist and pick up my diabetic pen, Belinda suggested I stay home relax she would return that shift before leaving for her home. Experiences are too many to share of encounters other careers and nurses and staff I have had with. Nurse Next Door are family, are you seeking such like people who care what more can be done to improve quality of life for everyone.
